what to tell IVF office?

I'm filling out my patient intake for IVF and it wants to know my pregnancy history... I tfmr for T21 in January and I don't even know what to tell them about that. I live in a very conservative anti-choice state and I'm scared. I'm also scared to lie about it because my regular obgyn knows I did this and I'm scared they will get my history from my dr office and see my nipt results, etc. Any advice?
