Manshire human dungeon and other t4, t5 dungeon entries

Hi,

so i head that there was some kind of bug which prevented the human dungeon in manshire from spawning a quest, but that this got hotfixed. Now i still dont get any quest for the dungeon on my latest runs. Am i missing something? Also i ran into some t4 dungeons with closed entries (checked one twice, lvl 5 and around 15 - still closed) Is this bugged?
