How badly did I F**k up?
Cll. Script was for 175. I put 75 in a labeled vial. Then I took off the cap of the sealed Cll bottle that contained 100. I put all in the bin (cap , sealed Cll bottle , labeled vial of 75). I assumed (which I know is wrong) that the pharmacist would see “okay there is only 75 in this vial, the sealed bottle must have 100, I’ll open the sealed bottle and put the 100 in the vial to complete 175.” I figured she did that because the vial clearly did not have 175. OR he would’ve asked me to print a label for the sealed bottle and label 1/2)..
Well, the pharmacist didn’t. He gave the patient the vial of 75. And now a week later the patient is saying he’s 100 short. Our inventory says we’re not short! So now I’m confused? My pharmacist is pissssed at me for not just double counting the sealed bottle and putting all in one vial..
Edit: is this a stars or verbal? To be more clear, no, the pharm does not double count C2s at my location. No I did not label 1/2,2/2. I was told the cameras are not helpful. . Should I text my pharmacist and apologize and ask what my punishment will be..
