Fraud on my account - again

I woke up this morning to yet another message that my account was compromised; this is the 4th time since the end of September.

In Sept, I got a call on my caller ID that said USAA, had all the right menus and asked for security codes...I fell for that one and that screwed me, but they didn't get money. Since then, I've changed my passwords, PIN's, cards, and I only call USAA through the app by requesting the phone number and clicking on it, I use the 6-digit security PIN when talking to them as well as phone PIN and phone password.

On Monday, I woke up to a message saying a new debit card was requested, but I didn't make it. I called USAA and the rep on the phone said it was being sent to my home address, confirmed the address with me and said it was the result of the potential fraud 2 weeks ago. I figured it was good...but on the phone right now, the rep said it was sent to Chicago and $603 was removed from my account this morning as well as a $580 attempted transaction which flagged and caused the review. The rep today escalated to a supervisor why the rep on Monday authorized a card to Chicago, but that won't do anything for me, just reprimand an employee.

Anyone else seeing fraud like this? I did a virus scan on my phone and computer - nothing found. I've changed security info 3x before today since the Sept fraud. It's getting to the point I want to pull out of USAA and go NFCU, but I don't want to lose discounts at USAA for my insurance

Edit: they are trying like hell, requesting 4x $600, 4x $580, 1x $560 and 1x $260 ATM withdrawals so far

Edit 2: they gained access to my accounts again and got a debit card for my savings account and pulled money from there. The reps today said they don't even know how that was approved for a card since it had never had a card before.
