Why are so many students below grade level?

Simple question, why do you think this is? What is happening in elementary schools that we have kids that can't do basic things in math, reading, or writing? It's not just one bad teacher one year causing it. It's a system failure? What are your theories? Or if you have concrete evidence please share that.
