Orders already picked up

Why am i allowed to accept/ given orders that have already been picked up by the time i get to the store? Then when i report the issue as a store issue, my acceptance rate sometimes suffers (not always and i have no idea what determines this). The order becomes a waste of time, gas, and my rating. What causes this?
